1. Introduction
These Terms of Service (“Terms”) constitute a legally binding agreement between you (“Client,” “User,” or “you”) and Revive Holistic LLC, a Florida Limited Liability Company (“Company,” “we,” “us,” or “our”), governing your access to and use of the website lotusmedcare.com and any services provided therein.
By submitting a request, making payment, or otherwise using our services, you acknowledge that you have read, understood, and agreed to be bound by these Terms.

2. Nature of Services – IMPORTANT MEDICAL DISCLAIMER
Revive Holistic LLC is not a healthcare provider and does not engage in the practice of medicine.
The Company does not:

• Diagnose medical conditions
• Provide medical treatment
• Prescribe medications
• Establish a physician-patient relationship
• Replace your primary care provider

Our services are limited strictly to:

• Coordination of laboratory testing
• Issuance of laboratory requisitions
• Administrative facilitation of informational consultations

Any consultation provided through our coordination is educational and informational in nature only.
Nothing on this website or in our communications constitutes medical advice.
You expressly acknowledge that you are solely responsible for obtaining medical care from a licensed physician of your choosing.

3. Client Responsibility and Medical Follow-Up
You agree and acknowledge that:

• Laboratory results may indicate abnormalities.
• It is solely your responsibility to consult with a licensed physician regarding any abnormal findings.
• Revive Holistic LLC assumes no duty to interpret results for diagnostic or treatment purposes.

If you believe you are experiencing a medical emergency, you must call 911 immediately.
You agree that you will not delay or avoid seeking professional medical advice because of information obtained through our services.

4. Eligibility
Services are available only to individuals:

• 18 years of age or older
• Physically located within the United States
• Excluding residents of New York, New Jersey, Rhode Island and Hawaii

5. Service Process
Upon receipt of payment:

1. A laboratory requisition is issued to the Client via email.
2. The Client independently schedules an appointment with Quest Diagnostics.
3. Laboratory testing is performed by Quest Diagnostics.
4. Results are delivered directly to the Client by the laboratory.
5. The Client must notify the Company to arrange consultation.

The Company does not control laboratory operations, processing times, or technical performance.

6. 30-Day Completion Requirement
Laboratory testing must be completed within thirty (30) calendar days from the date the requisition is issued.
Failure to complete testing within this timeframe shall result in:

• Automatic expiration of the requisition
• The service being deemed fully rendered
• No eligibility for refund

This provision is material and binding.

7. Payment Terms
All payments are made via Zelle to Revive Holistic LLC or using the direct payment link. 
Services are deemed rendered once the laboratory requisition has been issued.
No insurance billing is provided.

8. Refund Policy
Refunds are strictly limited as follows:
A refund may be granted only if:

• Laboratory testing has NOT been completed, AND
• The refund request is made within 30 calendar days of requisition issuance.

No refund shall be granted if:

• Laboratory testing has been performed
• 30 days have passed
• Client failed to attend appointment
• Client changed mind

Approved refunds may take up to 14 business days to process.

9. Limitation of Liability
To the fullest extent permitted under Florida law:
Revive Holistic LLC shall not be liable for:

• Laboratory errors
• Laboratory delays
• Misinterpretation of results
• Medical decisions made by Client
• Failure of Client to consult a physician
• Indirect, incidental, consequential, punitive, or special damages

Total aggregate liability shall not exceed the amount paid by Client for services.

10. No Guarantees
The Company does not guarantee:

• Accuracy of laboratory processing
• Medical outcomes or improvement of health conditions
• Prevention of disease

Laboratory services are provided by independent third parties.

11. Indemnification
You agree to indemnify and hold harmless Revive Holistic LLC from any claims, damages, liabilities, losses, or expenses arising out of:

• Your use of services
• Your failure to seek medical care
• Your misuse of laboratory results

12. Governing Law and Jurisdiction
These Terms shall be governed by the laws of the State of Florida.
Any disputes shall be resolved exclusively in courts located within the State of Florida.
