REFUND & CANCELLATION

REVIVE HOLISTIC LLC

At Revive Holistic LLC, we maintain a strict refund policy to ensure transparency.

1. Refund Before Testing
A refund may be issued ONLY if:

• Client has paid,
• Lab direction has been issued,
• Client has NOT completed laboratory testing,
• AND request is made within 30 calendar days of issuance.

2. No Refund After Testing
Once laboratory testing has been completed, no refund shall be issued under any circumstances.

3. 30-Day Expiration Rule
If Client does not complete laboratory testing within 30 calendar days:

• Service is deemed rendered
• Direction expires
• No refund shall be issued

4. No Refund for:

• Change of mind
• Personal circumstances
• Travel conflicts
• Missed appointments
• Failure to schedule lab visit

5. Processing
Approved refunds may take 5–14 business days.
